The Transition to Home program offered by Ambassadors is a home care for seniors in Houston service or program also known as Home Instead Senior Care. It is meant to help transition clients into a home from a transition care institution such as hospitals, rehabilitation centers and skilled nursing facilities. The transition is ensured with utmost safety and swiftness. Under the Home Instead Senior Care service, residents get drop-off prescriptions and pick-up medications and medication reminders. As far as their food and meal preparation is concerned, caregivers ensure adequate food availability and also prepare meals for the residents. Grocery is also shopped for if the need for it is expressed by the resident.

The caregivers are very particular about checking the expiration dates of medicines and cleaning up spoiled food. Our Transition to Home Program or home instead senior care is unique to Ambassadors Caregivers. This program was innovated to help transition clients home safely and swiftly from a transitional care institution like the hospitals, rehabilitation centers, or assisted living facilities.

Under the Transition to Home program, chores performed by caregivers include the following:

Meal Prep And Food Storage

Ensure adequate food is available, prepare meals

Grocery shopping if needed

Check expiration dates and clean out spoiled food

Chores and Supplies for Transition to Home Program

  • Change clothes for the day

  • Make sure necessary supplies are within reach, check air temperature

  • Food, water, snacks

  • Magazine, books, remote control, phone

  • Socks, slippers, bedside commode if needed

  • Set clothing out for the next day

  • Provide pet care

  • Retrieve old mail and help to sort out the mail

  • Empty trash cans and old garbage

Hygiene and Personal Care

  • Make sure there is adequate toilet paper, towels, bathrobe, shaving kit

  • Bath/shower supplies

  • Assist with bathing and toiling

  • Clear walk areas of tripping hazards: bedroom, hallway, bathroom, and kitchen

  • Make sure rugs are secure and not a trip hazard

  • Make recommendations for installations and removal of household hazards

The supplies delivered under the program include food, water, snacks, magazines, books, phone, remote control, socks, slippers, bedside commode if needed etc.

Hygiene and personal care items delivered include toilet paper, towels, bathrobes, shaving kit, bath and shower supplies and assistance with bathing and toiling.

Under the Senior Care specialty program we provide in Houston, safety checks done by Ambassadors Care include clearing all the walk areas such as bedroom, hallways, bathroom and kitchen of tripping hazards, ensuring rugs are secure and there’s not a trip hazard (by the recommendation of installations and removal of household hazards).
